The Rainbow Model of Integrated Care(RMIC) is a new conceptual framework that integrates primary care principles,integrated care characteristics,and a triple aim framework based on the Rainbow Model,which helps researchers better understand the concept of integrated care from a primary care perspective and thus scientifically conduct integrated care practice programs. This paper reviews the emergence and development of RMIC,its conceptual framework,and its application in integrated care,with the aim of providing a guiding basis for improving the quality of integrated care and positively transforming the health care delivery model in China.

Status quo and influencing factors of acceptance of advance care planning for young and middle-aged cancer patients

Objective To understand the status quo of acceptance of advanced care plan(ACP) for young and middle-aged cancer patients,and to analyze its influencing factors,so as to provide the basis for the implementation and promotion of ACP. Methods The convenience sampling method was adopted from September 2020 to February 2021 to investigate a total of 426 young and middle-aged cancer inpatients by a general information questionnaire,Patient’s Advance Care Planning Acceptance Questionnaire(ACPQ),Wake Forest Physician Trust Scale(WFPTS) and Death Attitude Profile-Revised(DAP-R). Multiple linear stepwise regression was used to analyze the influencing factors of ACP acceptance. Results The score of ACP acceptance of 426 young and middle-aged cancer patients was(66.30±9.32). Religious belief,course of disease,degree of pain,medical trust,and death attitude were the influencing factors of ACP acceptance of this population(P<0.05). Conclusion The acceptance of ACP in young and middle-aged cancer patients is at the upper-middle level and it is affected by many factors. Clinical medical staff should identify the relevant factors affecting the acceptance of ACP,take standardized measures to help patients correctly face the disease and death,and improve the acceptance of ACP. It is suggested to popularize ACP knowledge and try ACP for young and middle-aged cancer patients.

Progress on integrated care with quaternary linkage in senile dementia

With the acceleration of the aging process,the care of senile dementia has become the focus of atten-tion. This paper analyzes the concept of the Alzheimer’s disease,introduces the classification and diagnosis of Alzheimer’s disease,and summarizes the meaning of the integrated care with quaternary linkage. The development of integrated care were reviewed from the perspective of both at home and abroad,and its main framework and function were summarized. Recommendations on the network technology application,linkage referral,cultivating double levels of nursing talents,the foundation of Alzheimer community were put forward,in order to provide references for dementia care problems of elderly patients in China.
